What Are Mini Dental Implants?

The smaller diameter changes what they're useful for and where they fit. Like a standard implant, a mini implant integrates with the jawbone and supports either a single tooth restoration or, more often, a denture that snaps onto the implants.

The most common use case we see is denture stabilization. A patient with a lower denture that slips when they eat or laugh comes in, and we place four to six mini implants in the front of the lower jaw. Once the implants have integrated, the existing denture (or a new one) snaps onto them and stays put. No bone graft in most cases, no months-long waiting period, no large surgery.

Mini implants also work well in narrow anatomical spaces, most often the lower front teeth where standard implants sometimes can't fit between the natural teeth. And we use them for patients with low bone density who would otherwise need bone grafting before conventional implant placement.

Where Mini Implants Aren't the Right Choice


We'll be honest about the limits. Mini implants aren't the best choice for replacing a single back molar that handles heavy chewing forces – those teeth do better with a conventional implant that can take the load. They aren't usually recommended for full-arch fixed restorations like the All-on-4 treatment concept, where standard implants give a more durable result. And in patients with adequate bone for conventional implants, we often steer toward the standard option because it has a longer track record for single-tooth restoration of high-load teeth.

The Mini Dental Implant Procedure


The mini implant procedure is typically faster and less invasive than conventional implant placement, but the steps before and after matter just as much:

1.  Consultation and CBCT scan – We start with a cone beam CT scan that shows your jawbone in three dimensions. The scan tells us whether mini implants are the right call, how many we'll need, and where they should go. We review the images with you in the same visit. More on our implant technology and imaging.
2.  Treatment plan and sedation discussion – We map out where each implant goes, how many implants stabilize your denture or replace your tooth, and what sedation you want. We do most mini implant procedures under local anesthesia, but we offer nitrous oxide, oral conscious sedation, and IV sedation when patients want a deeper level of comfort.
3.  Implant placement – On surgery day, we numb the area, place the implants through a small opening in the gum (often without needing to lift a full surgical flap), and check the position against the plan. The placement itself usually runs 60 to 90 minutes for four to six implants.
4.  Immediate or delayed loading – In many denture-stabilization cases, we attach the denture to the implants the same day. In others, we let the implants integrate first and attach the prosthetic at a follow-up visit a few weeks to a few months later, depending on bone quality.
5.  Follow-up and long-term care – We see you back to confirm the implants are healing well, check the bite, and adjust the denture as needed. After that, we recommend a check at six months and then yearly, at either our Honolulu or Waipi'o office. Our post-op instructions cover the recovery week in detail.

The trade-off is that mini implants generally don't carry the same load capacity or single-tooth durability as conventional implants, which is why we use them where they shine and recommend single tooth replacement with standard implants for back teeth and other high-load applications.

How long do mini dental implants last?


In the right case, mini implants can last many years. Patients with denture-stabilization minis often go a decade or more on the same implants. Mini implants generally don't carry the same lifetime expectancy as conventional implants for high-load single back teeth, which is why we choose carefully where to use them. Long-term success depends most on case selection up front and on day-to-day cleaning afterward.


Are mini implants just cheaper, lower-quality implants?


No. Mini implants are a different size for different indications. They tend to cost less per implant because they're smaller and often need less surrounding surgery (no bone graft, smaller incision, shorter chair time), not because the implants themselves are lower quality. The titanium and the surgical technique are held to the same standards as conventional implant placement.


Can I get mini implants if I have low bone density?


Often, yes. That's one of the situations mini implants were designed for. The smaller diameter needs less bone width to anchor properly, so patients who don't have enough bone for conventional implants can sometimes skip the bone graft and go straight to mini implant placement. Does the mini implant procedure hurt?


During the procedure, no. We fully numb the area with local anesthesia, and we offer nitrous oxide, oral conscious sedation, or IV sedation if you want a deeper level of comfort. Afterward, most patients describe the discomfort as milder than a standard tooth extraction, often manageable with over-the-counter pain medication and ice for the first day or two.


Can my existing denture be used with mini implants?


Often, yes. If your current denture is in good shape and fits the arch well, we can usually retrofit it with the attachment housings that snap onto the mini implants. If the denture is old, ill-fitting, or worn through, a new denture made for the implants will give you a better result.
